Player Information and Waiver

This registration session will allow you to complete contact information, register you to your team, provide you with important Adult Hockey League information and complete the necessary waivers. 

Please read through each page carefully to ensure that all information is provided competely and accurately. 

 

ALL PLAYERS, GOALIES AND SUBSTITUTE GOALIES MUST COMPLETE THIS REGISTRATION IN ORDER TO APPEAR ON THE TEAM ROSTER!!

 

IF A PLAYER DOES NOT APPEAR ON THE ROSTER THEY MAY NOT PLAY IN ANY GAME.
